How weight loss injections work for men

What are they?

Mounjaro® and Wegovy® are both once-weekly injections that are approved in the UK for weight-management in eligible adults. 

How they work

These medications belong to the class of GLP-1 (glucagon-like peptide-1) receptor agonists (and in the case of tirzepatide, a dual GLP-1/GIP receptor agonist). They mimic the hormones released after eating. 

They work by slowing gastric emptying, reducing appetite and increasing feelings of fullness, which means you eat less and consume fewer calories, supporting weight loss. 

How effective are weight loss injections in men?

Clinical trials suggest that with medications like Mounjaro® (tirzepatide) and Wegovy® (semaglutide), when used alongside a healthy, balanced diet and exercise, many people can lose between 5%-20% of their body weight if taking the medication at the recommended dose long-term. 

These two medications act very similarly in both men and women, but there can be some potential differences that are specific to men, such as:

  • Men carrying excess fat often experience lower testosterone levels, sometimes referred to as obesity-related hypogonadism. While further research is needed, recent evidence suggests that some men taking GLP-1 weight loss injections or related anti-obesity medications may see healthy testosterone levels restored. In practice, this means that as well as weight loss, men using medications such as Wegovy® and Mounjaro® may see some benefits in libido, muscle strength, mood and energy.

  • Men typically accumulate more visceral (abdominal) fat than women. This type of fat is closely linked to diabetes, cardiovascular disease and low testosterone. So, for men especially, reducing waist-circumference and visceral fat is often a key goal for its health benefits. One of the things that weight loss injections are most effective at is reducing visceral fat and helping to improve overall metabolic health.

How are men prescribed weight loss injections?

  • A qualified prescriber assesses your eligibility and personal suitability for this type of medication. This may be a weight management professional that your NHS GP refers you to, or you may opt to be assessed for a private prescription.

  • Once prescribed, you use the injection typically once per week, alongside a healthy and balanced diet and active living.

Which men can take weight loss injections in the UK?

In order for men to be prescribed weight loss injections in the UK, there are various eligibility requirements. These depend on whether you are accessing the medication via the NHS or through a private prescription.

NHS eligibility (England)

  • Mounjaro® for men on the NHS is currently being rolled out with strict criteria. You must be an adult (18+), with a BMI of 40 kg/m² or more, and at least 4 weight-related health conditions (such as type 2 diabetes, high blood pressure, heart disease, obstructive sleep apnoea, abnormal blood fats). 

  • Wegovy® for men on the NHS follows slightly different criteria. You’ll typically be referred to a specialist weight-management service and will need to meet eligibility requirements such as BMI 35+ (this can be lower for some ethnic groups with an increased risk or certain conditions) plus a weight-related health condition. 

Private prescription eligibility: weight loss injections for men

Weight loss injection prescriptions for men who meet clinical eligibility (obesity, weight-related risk) after an assessment can be accessed privately through an accredited prescriber. It’s important to note that only a qualified UK prescriber can legally prescribe these medications.

Who may be eligible for private weight loss injection prescriptions?

For both Mounjaro® and Wegovy®, you may be eligible for a private prescription if you:

  • Have a BMI of 30 or higher

  • Have a BMI of 27 or higher and also have a weight-related medical condition, such as high blood pressure, prediabetes, high cholesterol or heart problems.

 It’s important to note that weight loss injections are not suitable for all men, even if you meet the above criteria. This is why an assessment is needed to ensure that it’s a safe and appropriate medication for you specifically.

Are weight-loss injections safe for men?

When prescribed appropriately, used under medical supervision and with the right lifestyle support, weight loss injections like Wegovy® and Mounjaro® are safe for many eligible men to take. There are some side effects that many people experience when taking these medications, but they usually reduce or go away entirely as the body adjusts. 

Typical weight loss injection side effects

Men should be aware of the possible side effects of these medications, including:

  • Digestion related side effects, like nausea, vomiting, constipation, diarrhoea or stomach pain.

  • Headaches

  • Fatigue

  • Rare but serious side effects, such as pancreatitis (inflammation of the pancreas).

FAQs about weight loss injections for men

  • There is no current evidence showing a consistent gender difference in weight-loss outcomes with tirzepatide (Mounjaro®) or semaglutide (Wegovy®). Clinical trials report overall average weight-loss percentages (e.g., up to ~20%) for both men and women, but individual results do always vary. 

  • The opposite is often the case. Emerging research suggests that anti-obesity medications including GLP-1 agents, like those in weight loss injections, may increase testosterone levels in some men with obesity or type 2 diabetes. It should be noted that this is still early research, and individual monitoring remains important.

  • There is not currently any evidence of weight loss injection side effects that are specific to men and don’t affect women taking the same medication. The common side effects (nausea, gastrointestinal symptoms, risk of pancreatitis) can be experienced by anyone taking weight loss injections. Most side effects usually go away once the body adjusts to the medication and dose.

  • Generally, men aged 18+ with obesity (BMI criteria as per NHS/privately) and weight-related health conditions may be eligible for a prescription. However, not everyone is suitable for taking weight loss injections and a full assessment must always be carried out by a qualified UK prescriber before you start taking this medication. 

  • Clinical trials indicate that men taking Mounjaro® can lose somewhere between 5%-20% of their body weight, over a period of up to 18 months. It’s important to note that everyone responds uniquely to medication and actual results depend on adherence, starting weight, fitness, muscle mass, metabolic health and lifestyle.

  • It’s essential for men taking weight loss injections to use them in combination lifestyle changes, such as high levels of activity and a balanced diet. Maintaining these healthy habits will help to sustain the weight loss achieved and not regain it if you stop taking the medication. 

  • Rapid weight loss can affect both fat and muscle. Men using Mounjaro® or Wegovy® may notice changes in strength or exercise performance, especially if appetite has reduced significantly.

    To protect muscle during weight loss treatment:

    • Eat enough protein throughout the day

    • Lift weights or use resistance training to maintain strength

    • Talk to your trainer (if you have one) about adjusting your routine

    • Give your body time to recover between sessions

    • Avoid skipping meals

    Most men find that as body weight drops, body composition improves even if absolute strength changes slightly.

  • It can be more challenging! This is often due to changes in metabolism, stress, lifestyle and hormone levels. Weight loss injections, combined with activity and a balanced diet, can help support progress at any age.

  • Many men with obstructive sleep apnoea carry extra fat around the neck and abdomen, which can make the airway narrower and breathing harder at night. As weight comes down - especially visceral fat - the pressure on the airway often reduces too. This is why some men may notice they snore less, wake up fewer times and feel more awake during the day once they’ve been on Wegovy® or Mounjaro® for a while.

    It’s not a guaranteed fix for everyone, and it doesn’t replace your usual sleep apnoea treatment, like CPAP (so it’s important to keep using them). But losing weight can make a real difference to how well you sleep and how refreshed you feel. If you have sleep apnoea, always consult with your GP or sleep specialist as you go, so they can guide you safely as things hopefully improve.

  • There’s no evidence that Wegovy® or Mounjaro® harm male fertility. In fact, for some men, improving overall health and reducing excess weight can have a positive impact on things like testosterone levels, energy and general wellbeing, which all play a role in reproductive health.

    If you’re having fertility checks or you and your partner are trying to conceive, it’s worth mentioning this to your prescriber so they can give advice based on your individual health and any other medications you’re taking. But for most men, these injections don’t cause fertility problems and may even support better hormonal balance over time.

  • If you are training or aiming in your gym activity to build or maintain muscle, there are some important considerations if you’re thinking of starting weight loss injections. 

    • Appetite may reduce, so ensure your diet includes enough protein to preserve muscle.

    • Because you lose fat mass, your body weight may reduce and possibly strength/performance in absolute terms will adjust. Body composition should improve. 

    • Monitor your fatigue after workouts or training sessions to ensure recovery and speak to your trainer about adjusting load and nutrition.
